May 2024 Half Term Booking Form
Dear Parents and Carers
Please find attached the booking form for our May Half Term 2024 holiday provision, please complete your booking form and submit it via email stating the method of payment and providing proof of payment with your booking form.
Please await a confirmation of your booking before making any payment
Please note we are unable to refund voucher payments but can offer credit for usage. We require 7 days notice of cancellation, notice of cancellation needs to be emailed to kingsheathplaycare@gmail.
We will be delivering a full fun filled provision over the 4 day week operating from the large school hall, with drop off and collection at the main entrance of the school. We will be providing breakfast and high tea for the children and offering different activities along with arts, crafts and plenty of outdoor activities.
Please can we remind parents we are a 'Nut Free' provision due to children attending with severe allergies, please bear this in mind when packing your child's lunch for the day.
Should your child be accessing us for the first time please complete the attached registration and consent form. If you are a new user, please contact us via phone (0121 444 5826) to discuss any requirements and payment arrangements.
